Candidates appearing for JEE Main 2025 have to fill out the self-declaration form. The form is part of the admit card & must be filled out before arriving at the exam centre.
The self-declaration form is an undertaking that the candidate has read all the instructions and guidelines and will adhere to them. It is part of the second page of your admit card.
This form ensures that the candidate understands the exam instructions & is prepared to take the exam. It must be filled before the exam but signed in the presence of the invigilator.
Visit the JEE Main website: jeemain.nta.nic.in.
Click on the "Download Admit Card" link.
Fill your application no. & DOB.
Your self-declaration form will be add in the admit card.
Before filling out the form, read all the instructions carefully. Make sure to fill in the correct detail in the space provided.
There will be a section to paste the candidate’s photo. Affix the same photograph as uploaded in the application form.
Candidates have to put their left-thumb impression on the self-declaration form before reaching the exam centre. Make sure you put the impression clearly and only once.
There will be space provided for your signature. Candidates have to sign in the given space on the day of the examination in the presence of the Invigilator only. Ensure that you do not sign before reaching the centres.
