If the designation of the Issuing officer isn't provided in English, then I would recommend you to write the term written in Hindi in English as you have done in the question. This way you will provide the designation and no problem will occur. Now I would recommend you to contact the NEET UG helpdesk and get it discussed and after that only fill it. This way you would be sure.
