your school ID card is not needed for anything. Documents that is required for counselling sessions are;

• 10th and 12th mark sheets
• 10th and 12th passing certificate
• Rank card and score card of NEET 
• Neet counselling Letter 
• birth certificate
• caste certificate

As per the rules given for JIPMER 2018, name and date must be written on the photograpgh of candidate. And there is no facility of corrections for application form. There will not create any problem. But you should contact once so that it will not create any problem.
