Do I have to fill state LEVEL form too for state rank in NEET 2019

You need to apply only NEET Application form now, After NEET result the particular state counselling authorities will start its registration, at that time you need to do the registration for 85% quota seats, The state you belong with your NEET Rank/Score.

It depends from which state u are .
Some states do have a separate counselling form for there state .
If there is some form like that then u will have to fill it to get your state rank and college in your state under 85% quota .
Neet only deals with 15% seats in colleges of all India .
So whenever the state form comes out fill it with accuracy .
