Add surname on passport. If eligible for cat test

I believe you have made an amendment in your passport by adding your surname. Since you are required to carry a photo ID proof the CAT 2018 admit card, I suggest you carry both - your passport and admit card on the day of the exam. So with that, I believe that the issue is resolved if admit card and passport have the same name.
However, if your name on admit card and passport do not match, then you must take supporting documents for the name change at the time of examination, interview and admission.

In case you have no supporting documents for the name change, you stand the risk of not being allowed to appear in CAT 2018.
