I am qualified for jee advanced.,but I do not want to fill the form or appear in exam.then will i lose one chance for advanced or not.

AS you mention above you have only two chances for appearing on with 12th and other chance will be after 12th. if you miss that chance miss you lose your chance and you left only one chance if you are 12th appearing or if you passed out 12th last year then there is no chance left for you. sorry to say but you lost this chance.
