i dont have community certificate for caste certificate what should i do where do i get sangam certificate for BCA

Being SC category candidate and with this score you have high chances of getting mbbs but which college you will get can't be predicted now. Competition is increasing day by day. It depends upon cut off Neet 2024, cut off changes every year and it depends upon various factors like Number of students appeared in exam, Toughness level of exam, seat intake, normalisation process and previous year's cut off.
Qualifying percentile For neet for sc category is 40 percentile .

Mam I have got 138 marks is bhms possible in this college?

You have just qualified neet , 138 marks are at the border line.
You haven't mentioned your category to which you belong so considering you as a general category candidate it's difficult to get bhms seat .
Neet qualifying percentile For general category candidate is 50 percentile ie score between 720 - 137
Also it depends upon cut off neet 2024. Cut off keeps changing each year and depends upon factors including number of students appeared in exam, number of seats available , normalisation process, difficulty level of exam and previous year's cut off etc.
So looking at previous year's cut off trends , a general category candidate must score minimum 300+ to get a private college for bhms .
