Hi aspirant,

You should fill the name as on yours and her documents. See you can apply for corrections in your marksheets and get it updated by the time of counselling. Even in case you are not able to do so, you can carry an self attested affidavit claiming that both the persons are same and irregularity in name is due to virtue of unintentional mistake.
