Bhai,you don't need to be panic, you can read the FAQ section of jee main website. If you have forgotten to submit your name and date it will be ok.

No it's not alright, you have to correct it on given correction date by JEE mains .
The date are appeared on the official site of JEE mains.
Do check and don't take it lightly

The form asked me to upload a image , then the signature of the student And Father , , now does it necessary that the uploaded image should contain the signature and name ?
No it is not fully mendetory .If you have forgotten to submit that it will be ok.
