see generally in different recruitment or admission test counselling or document verification procedure the name which is in 10th marksheet is taken as reference and it is expected to be same in all the forms and application you may fill in future. however you have to maintain the only one name in all of the forms and application. however it would not really matter if you use the name as in 10th certificate.
