Is certificate(PWD) is required to show at exam center of jee mains? and the format of certificate is different from the original certificate issued by government?

Yes if you are a pwd candidate then you have to show the original certificate at the exam centre because pwd candidate are given a separate room than other students, so the certificate that you have you have to bring both the original and photo copy of it, during the time of examination.
