    Hello student ,

    As you have not mentioned about which exam you are asking for the self declartion form was attached with the admit card or not . That why I am not unable to guide you with your query . As you know different exam has different pattern of attaching self declartion form with the admit card . So I would like to suggest you to do mention about which exam you are asking for .
