Considering your situation, if you have an option of editing the form you may delete the Declaration form and upload the EWS certificate.

If there is no option to edit your application form then you can write a mail to the organization/institute and attach a neatly clicked image of your EWS certificate. Usually, the certificates are verified at the time of counselling.
