- Provisional Allotment Letter generated through Online
- NEET Hall Ticket/ Admit card (Original)
- NEET Score Card
- 10th standard Pass Certificate for proof of date of birth (Original)
- 10+2 Marksheet & Pass Certificate (Original)
- Transfer Certificate or School/College Leaving Certificate issued by the School/College (Original)
- Conduct/ Character Certificate issued by the School/College (Original)
- Four recent passport size photographs
- Aadhar Card Copy
- One set of photocopies of all the above documents
- Reservation Category Certificate (if applicable) (Original)
