Self declaration for covid-19 form of TS EAMCET exam 2021 will be filled up by the candidate before entering the exam hall. This form will be filled up with hall ticket number, personal details and exam date in the self declaration form of TS EAMCET 2021.candidate also need to fill up the details of covid-19 symptoms if any in the self declaration form of TS EAMCET 2021.
