If you hold the domicile of West Bengal you will need any of the A1, A2 and B certificates. If you do have them, then you do not need the proforma that is available online. The original domicile certificate should be good enough. Other documents that you will need are:

• Acknowledgement slip
• Voter Id/Aadhar Card
• Caste certificate if applicable
• NEET 2020 Admit card
• NEET 2020 score card
• PWD Certificate if applicable
• Proof of fees payment
• Class 10th marks sheet
• Class 12th marks sheet
• Proof of Date of Birth(Your 10th marks sheet should be enough)
• Allotment Letter
• The verification slip that you have generated during the Pre counselling phase
• Document Verification Slip
