If you hold the domicile of West Bengal you will need any of the A1, A2 and B certificates. If you do have them, then you do not need the proforma that is available online. The original domicile certificate should be good enough. Other documents that you will need are:

Acknowledgement slip
Voter Id/Aadhar Card
Caste certificate if applicable
NEET 2020 Admit card
NEET 2020 score card
PWD Certificate if applicable
Proof of fees payment
Class 10th marks sheet
Class 12th marks sheet
Proof of Date of Birth(Your 10th marks sheet should be enough)
Allotment Letter
The verification slip that you have generated during the Pre counselling phase
Document Verification Slip
