Since you have not mentioned the name of examination for which you have filled the application form so it is little bit difficult to guide you accordingly.

Generally some examination authorities provides facility of correction window if they also do so than you can correct your name there. If they don't provide correction facility than you have to directly contact them through mail or contact number whichever is provided on the official website.
