If I have registered for UP 1st round counseling but was unable to report to the respected nodal centre for document verification and also not locked the choices. Will that registration work for the second round counseling or we'll need fresh registration for UP second round counseling?

It is rather difficult to say as some states would not allow a candidate for direct counselling in round 2. Incase you were allotted a seat and you were unable to reach there then I would suggest you to talk to the authorities as often they extend the time so that might help you. And in other states which will allow you in those cases your old registration details would work. For that I would say keep an eye on the official website regarding any updates.
