As per mci guidelines, 

The provisions of the said Act require that person with benchmark disability (who has a minimum of 40% of a specifieddisability) is entitled, as a matter of right, to avail all the benefits and accommodations enshrined in the Act including 5%reservation in higher education 

Basically i would say that to know whether you are applicable for neet counseling 2019,with disability ,go through the government notice in official website.

There is reservation for physically handicaped students in Neet.The percentage of disability should be 45 percent.
