You have not mentioned which category you belong to. With a score of 645, even if you are in general category, you can get a seat in B. J medical college, Ahmedabad. Last year,  a score of 620 got a seat in BJ medical college under the UR category in all India quota. But one thing to keep in mind is that scores and ranks vary each year with difficulty level of the question paper and availability of seats. Dear Shivam, you haven't mentioned your Category. Neverthless, because you belong to Rajasthan and want to apply for a college in Gujarat, you should apply under All India Quota. In 2018  under All India Quota, the cut offs for various Categories have been:

General Category: 620
SC: 492
ST: 478

With 645, you can get a seat there.  Having said that, cut offs change every year and are dependant on various factors like no.of seats available, no.of candidates appearing for the exam, top NEET score and difficulty of the exam. So we would suggest you to wait until the official answer key has  been released to weigh your options.
