Yes you have to paste your passport size photograph on your NEET admit card .Don't forget to carry your ID proof with you to the examination hall as it will be required for validation of your identity.For ID proof you can carry Aadhar card,PAN card or voter ID as without ID proof you won't be allowed to enter the examination hall.
