In my admit card of jee main ,in candidate name my surname is not given so what is the procedure to put surname? is there any email or phone number for it?

You should reach out to the helpdesk at the email available on the admit card. For your convenience, here it is; jeemain-nta@gov.in (mailto:jeemain-nta@gov.in) . Please mail them from your registered email and they will make a note of it and let you know the next course of action. Also attach a screenshot of your admit card and that will be much easier.

But Jan 2020 exam the correction date was till 20 Oct 2019.
You can give a try like contacting through phone or email.
I think you have no chance after the correction date but give it a try.
