You haven't mentioned your Category and so it is difficult to predict your chances. Nevertheless, this answer will be written in the perspective of General Category as they have the highest cut offs. In 2019, under State Quota, 534 marks secured a seat in Mysore Medical College under General Category. Compared to 2019, the NEET 2020 cut offs have changed considerably higher than expected. With the top score being 720 which is 19 marks greater than 2019's top score, it is difficult to predict chances. But with 613, it is easier for you to get a seat in Mysore Medical College, irrespective of your Category. Having said that, this is based on 2019 data and cut offs change every year. Cut offs change every year depending on various factors such as no.of Candidates appearing for the exam, no.of Candidates qualified, top NEET score and difficulty of the exam. First of all heartiest congratulations for getting such excellent marks and hope you are doing well.

At your rank you will easily get  government colleges through karanataka state quota. Also KEA does not prepare any state rank they take counselling through the AIR only till now. You will also get Mysore Medical College, Mysore very easily. Last year cutoff for this college through karnataka state quota for general category was AIR 31009 (534 marks) .
