Yes, you are eligible for Neet 2021 examination with your date of birth on 29th December 2004

This is because,

To be eligible for Neet 2021 examination one needs to be at least 17 years old by 31st December 2021

And you fulfill this criteria  with your date of birth on 29th December  2004  as
you will be 17 years old on 29th December   2021 itself i. e you will be 17 years old before 31st December 2021 and thus eligible for Neet 2021 examination with your date of birth

Also,

To be eligible for neet 2021 one must be either appearing for  12th board examination 2021 or should have passed 12th board examination with physics, chemistry ,biology/biotechnology and English and must have got at least  50  % in aggregate of PCB  i.e Physics, Chemistry and Biology /biotechnology in 12th examination  if belongs to general category , or   at least  40%  in aggregate of PCB if belongs to  Obc/ Sc /st category.

Now that you are eligible with your date of birth, and you  have limited time left for your neet examination I'll suggest you to focus on your preparation and revision,  go through mock test paper sample papers and previous year question papers as it will let you know your preparation level and and weak areas so that you can work on them and perform well in your examination.
