in the official website neet.nta.nic.in, the information bulletin contains the format of the affidavit within it.

Hi Sampriti!

Yes, you need to submit the affidavit for single girl child while filling the application form for NEET 2021. The option will be available, if you choose gender "Female" and you do not have any siblings(brothers/sisters). Kindly note that you nees to submit the affidavit in the format as mentioned by the NTA. You can check out the format here:

But when i had applied for neet 2020 there was only the option whether a single girl child or not and i had to select only yes or no, there was no option for uploading of the affidavit. the link that you have shared is information bulletin for NEET 2020.
