• KEA application form and verification slip

• KEA allotment letter

• Fee payment receipt

• Class 10 marks card (for date of birth proof)

• Class 12 marks card and pass certificate

• Transfer Certificate from last studied institution

• Migration Certificate (mandatory for non-Karnataka board students)

• Eligibility Certificate from Rajiv Gandhi University of Health Sciences (required for non-Karnataka students)

• Study certificates (Class 1 to 12)

• Character and conduct certificate

• Aadhar card of student and parents

• PAN card of parent
