Hi , talking about declaration form go with the one that you have downloaded along with your admit card . Paste your photographs where required and take few photographs so as to paste them at examination centre on attendance form too . Don't forget to carry your masks and gloves along with 50ml sanitizer bottle as they are mandatory keeping the current pandemic situation in mind.
