It is mandatory to fill your given name and surname while filling the passport application because Some Embassies (Embassy of U.S.A., etc.) insist on surname for issue of visa so it is better to include Kumar if it is your surname. Before applying for Passport please correct the name given on Adhaar card. You can correct it by visiting the nearest Aadhaar Enrollment center and filling the application for correction. You will obtain updated Aadhaar card within 10-15 days from the date of application. Now Adhaar is not the only document you can enclose with the passport application you can enclose other valid documents as proof of address such as water bill, electricity bill. But enclosing Aadhaar card will make the entire process of passport application hassle free. So if you do not want to have issues at the time of appointment please apply for name correction in Adhaar Card. And mention your given name and surname while filling the application form for passport.
