As per your requirement, i would say that no, you dont require to do fresh or basic registration again.
The basic registration stays there. Whether you appear for this exam or not, the next time you register for final registration, you can use the same basic registration credentials. So the time whenever you want to attempt AIIMS PG, you can jump directly to the final registration process without doing the basic registration again fot aiims pg exam.
