If you want to issue Debit Card from your bank where you have the account, you need to write a letter to the Branch Manager stating that you want to issue debit card facility and also add your name and account number in the main body of the letter. In some banks, a form called multipurpose application form is available through which you can apply for your debit card. For this you need to do nothing, just put your address, name, and signature in the form at the required area. And tick the line there, where it is written about application of debit card and submit it to the service manager or anyone of that bank.
