As you have not provided enough details like on which certificate you want to change your dob and name what's your current age and class because if you have passed your 10 examinations then there is no any way to change your dob now but if you are asking for correction in dob in any state issued document then you can apply for a new certificate . You can change your name if you have not passed 10 till now because after 10 it will create problem for you if you have different name on your 10 and 12 mark sheet

If you are still in lower classes you can contact your school authority .you will have to fill an application form with proper documents and your name and dob will be changed.

If you had put you dob wrong on any  college application from you can wait till they issue their correction form.

Just explain your problem to your school Headmaster that you have this problem in your mark sheet.

• You will be given an application form for correction of name in your certificate.

• You have to forward various documents related to your admission in the school
with this application like admission form which was filled during admission into the school, school leaving certificate and other relevant documents related to your admission which mentions your correct name.

• This application will be forwarded to the board by your school headmaster and if your documents are valid then there will be no problem in correction of your name in the marks sheet.
