You didnt mention for which purpose do you need PWD certificate for. But generally PWD certificates are given by District Government Hospitals. You can apply for Unique Id at: https://www.india.gov.in/spotlight/unique-disability-id . A Chief Medical Officer or Sub Divisional Officer of the District  or an expert in the Specified field form a Medical Board and any one from this Medical Board alone can give you a PWD certificate. You can visit any specialist for conditions like ENT. Their Certificate is valid. But for other disabilities, you will need to get it from the Medical Board.

Parent of a person with disability, or a person himself, should approach to the District hospital with his request for obtaining Disability Certificate providing the following documents:-

1.Copy of I.D. of the person with disability and 2 photographs showing the disability part.

2.Copy of all medical and psychological reports available
