Taking consideration of lockdown pass, there are online portals where you can apply for pass and after filling all the required details and verification your pass will be available to download.

If there is no online portal, than you can write an application to the collector ot your district. Application must include sender's address, date, salutation, post(here District Magistrate), subject, main body with genuine reason. End it with proper thanks and giving your contact details.
