Once you get a good score,

Ex : In CAT then you'll be automatically get a call from IIM's as it includes in their fee.

For other top b school, you need to apply separately to their website and fill their application Form. Each top B-School like SP JAIN, VJSOM,IMT -G opens there form once exam score are out and you need to fill it by there respective deadline.

Further after scrutinizing your score and parameters set by those top b schools , you'll be shortlisted for GD PI process

After that a cumulative score is being calculated for your Graduation+ 10+12+work ex if any+Test score + GD+PI thereby a merit list declared.

Well to start with after getting your scores, compare it with the cutoffs and then we need to decide what college you need to take admission into.
Now to opt for college you to apply for every college separately, by downloading their forms filling them up and them submiting. Later when you are shortlisted you go under various process of selection.
1. WAT
2. GD
3. PI

Once you qualify all these stages of selection you are admitted into the college.
Whether the college be top most college like IIM or a normal local college the process is the same.

Yes you have to apply for each college you wish to join. However you have to check the cut off then proceed further with filling up the form and payments.

Check the criteria for applying for the colleges, make a list of colleges, then start preparing for WAT, GD/PI.  And you also need to check on which entrance exam basis the college takes admission like some colleges only prefers CAT exams whereas some colleges prefer CAT,CMAT or both. And there are colleges which conducts its own exams, so they prefers that scores only.
