I think you are talking about change in 10th marksheet, if it is so then it is not possible to change your name in either 10th or 12th marksheet. You can add an affidavit which mentions that you have changed your name. This is the only option for name change you can have, you can even contact the authority which issued you the certificate they can guide more if it is possible.
