You can get the medical certificate by visiting the official website of JOSSA at https://josaa.nic.in/WebInfo/Page/Page?PageId=1&LangId=P , click on the link of Business Rules, download it, you will be able to see the Medical Certificate to be issued by Registered Medical Practitioner on annexure 7, at page 61.
