You need to do the following procedure:

Applicants must apply online and pay the required fees in order to be considered for the Provisional Statement of Eligibility.
In order to apply for the Provisional Statement of Eligibility, candidates must register. They will receive a login ID and password upon registration in order to submit their application online.
It is mandatory for candidates to upload their marksheets, NEET, JEE, and NATA if relevant.
Candidates who pay the required fee may apply for up to three programmes.
After paying the required fee, candidates may submit additional applications for up to three programmes.
Candidates can download the Provisional Statement of Eligibility Certificate to be submitted to the appropriate authority after it has been approved by the university.
