To get a medical fitness certificate , there is a proforma.

With the proforma, go to any govt hospital or any registered medical practitioner in your area, they will check you according to the proforma and then provide you the medical fitness certificate with stamp and their registration number.

This certificate is needed at the time of verification.

Medical fitness certificate will only be given in the registered hospital by a registered medical practitioner (at least an M. B. B. S).
