You have to go to your bank and ask for an application form for debit card, fill it  attach your ID and address proof and submit it to the branch.

You'll get it after about 7 days of Submitting the form.

In private banks you can get an instant card too which will be activated in 24 hours.

You can also submit an online application for the debit card.
