you will find medical format in upsee brouchure 2018. visit upsee website and download brochure and it is found in last pages.

But on the medical certificate the date is 2017

YOU MAY TAKE ANY GOVT HOSPITAL AND ANY DISTRICT.

There is lot of corruption in Govt medical hospital . Without service tax none of the person talk's in right manner .

Sir medical certificate apane his district hospital se banega ya kahi aur district hospital se banawa sakata h

Youll get the format in the brochure. Also, you can get it at Chief Medical Officers office.
after general check-up you can get
