To be eligible for neet 2021 one has to be at least 17 years old by 31st December 2021 and with your date of birth on 21st July 2004 you fulfil this criteria as as you will be 17 years old on 21st July 2021 itself i.e before 31st December 2021 and so yes with your date of birth you are eligible for Neet 2021 examination

Also, to be eligible you must have pass in English and in both the theory and practical of PCB i.e Physics, Chemistry and Biology with at least 50 % in aggregate of PCB ( in theory +practical) in 12th examination if you belong to general category , or at least 40% in aggregate of PCB if belongs to Obc/ Sc /st.
