As per your query , I would glad to inform you dont need to worry at all ,if you have registered once for phase  1 registeration then  you dont have write down in neet phase 2 registeration . Moreover ,you have to be present with all the marksheets of 10th and 12th at the document verification time ,so you dont need to worry at all
