You do not have to upload class 11th certificate in NEET UG application form. The list of documents that you need to upload in NEET UG application form are given below

• Class 10th certificate
• Candidate's passport size photo
• Candidate's Postcard size photo
• Left and Right hand fingers and thumb impressions
• PWD certificate (if applicable)
• Candidate's signature

These are the documents that you need to upload. In Class 11th Education details you have to provide your details such as pass status, year of passing or appearing, qualifying exam, roll number, type of school, place of school, result mode, address of the school etc. Please check your class 11th hall ticket to find your roll number. If you do not have any hall ticket number then you can enter your roll number which was allocated to you when you were in class 11th.

If your 11th examination is conducted by your school, then I would recommend you to visit your school and apply for the marksheet of 11th. In many schools, the 11th marksheet aren't given, but if you apply for the marksheet then the school will issue a marksheet for you. Contacting your school is the only way out.
