Dear charan kumar, You need following certificates for income certificates:

1 You need address proof like Copy of Voter Card / Name in the Voter List 
take one of them and attached it with application form 

2 Make a report of you income and verify it by Patwari

3 Write a letter in which you are self declaring that you have this income annually 

4 For birth of date verification attached a school certificate 

5 If your father is a farmer than there is not need of salary slip
