Dear Sonu, Yu cannot make changes to your photograph. Having said that, not mentioning name and date on your application doesn't call for a rejection of the application. So, you are fine even if you do not have name and date on your scanned and uploaded photograph. Your form will be accepted if other fields of the application are intact.
