Guardian is anyone who is responsible for handling your finances and welfare till you turn 18 and your  parent is considered as your natural guardian. At the time of documents being made, your father was your guardian so his name is there as guardian on your documents but just because he has separated now and is no more your guardian doesn't mean that your documents have become invalid. Your documents are still valid.
While filling the neet application form if you have to fill your guardian's name then instead of your father, you can definitely fill your mother's name and then you won't face any problem.

Authorities don't ask you for your parents documents, rather it's you whose documents are verified.

You will need the following documents for Document Verification :

*your NEET Admit card
*your  NEET  Rank Letter /score card
* provisional Allotment Letter
*your Class 10th mark sheet and certificate
* your class 12th certificate and mark sheet
*TC
*Domicile certificate ( in case of state counselling)
*your 8 passport size photographs
* your photo id proof ,like aadhar card or pan card or passport  or driving license
* your caste certificate ,only if it is  applicable
*pwd certificate ,only if it is  applicable
