Abhinav, your 10th marks sheet or passing certificate will act as the proof of your Date.of Birth and so even while filling in the NEET application form, you need to mention the Date of Birth according to your 10th marks sheet and not your Birth Certificate.

If you look at the documents required for counselling and document verification, it says that 10th marks sheet is the proof for date of birth.
