first i'd suggest you too be attentive while uploading docs and information with forms. no kindly check through the id and pass if you have an option to change the uploaded information, which i think is not possible. you can either mail the concerned department or cancel the form and fill other one as it will actually hamper your seat allotment on the basis of feeding them with wrong info.
