Flow chart is the best way to revise a chapter but before making a flow chart make sure that you understood the chapter very well otherwise the flow chart is not going to help. you can write a little bit details side of the chart so that you can easily understand the concept. Place the chart on the wall straight where you study and try to see and learn them whenever you see them. For maths instead of mugging the formula try practice questions and then learn them during practice it will last long same for Physics.

The best way to remember your notes is just to solve the as much questions as you can. 30 years previous question papers are enough, if you have already done those questions, then repeat them, solve them again till the exam.
