You don't need to worry about that your form won't be rejected.
Including your name and DOB on photo doesn't fall in the category for discrepancies so you need not take tension.

It,s ok. If it is rejected then you will receive a mail from PAAR authority mentioning to re-upload the incorrect image. You can correct the details from 17th January 2019 to 22nd January 2019 in the AIIMS Application Form 2019. For more information, please go through the link given below
